KEEP YOUR LATEST ANNUITY ADJUSTMENT FORMS
It is that time of the year when we receive the Annuity Adjustment forms.
Normally we receive two of these each year, one in late December for the Cost of
Living Adjustment (COLA) and one in January that reflects the change in Federal
Employee Health Benefits (FEHB). I recommend that you file the last two of these
Annuity Adjustment forms that you receive each year with your important papers.
They have a lot of good information on them:
your CSA or CSF number, details about your annuity, the toll free number and
address for the Office of Personnel management (OPM), procedures for notifying
OPM of deaths, changing your address and how to contact OPM. Also detailed
information about survivor benefits including for the spouse you were married to
at retirement as well as for a spouse you marry after retirement.
For those who have provided benefits for a survivor it also shows how that
benefit will be affected by the COLA. By keeping the last two of these Annuity
Adjustment forms you or your survivor will have up to date information when the
need arises.
